The Trial That Changed the Conversation

SSaSS randomized roughly 21,000 Chinese adults with a history of stroke or age over 60 with high blood pressure to either regular salt or a 70/30 sodium-to-potassium chloride substitute, used for all cooking and seasoning for five years. The results, published in the New England Journal of Medicine in 2021, were substantial: about 14% fewer strokes, 13% fewer major cardiovascular events, and 12% lower all-cause mortality in the substitute group, with no observed excess of serious potassium-related harm.

The honest framing writes itself: this is the strongest trial we have, in a population with very high stroke rates, using an aggressive 70/30 substitution, and the effect was visible at the population level. Who Should Not Use These Products

Potassium chloride is medicine for some people and a hazard for others. Hyperkalemia — blood potassium above the safe band — can develop quietly and is a genuine emergency when it arrives, so the exclusion list is not negotiable-by-feeling: if you are on it, substitutes are off the table without clinician approval.
