What Kefir Is, and Why It Outguns Yogurt

Kefir is made with "grains" — rubbery clusters of bacteria and yeasts held together in a polysaccharide matrix called kefiran. A scoop of grains ferments milk at room temperature for about a day, producing a thin, lightly carbonated, sour drink. Yogurt, by contrast, is set by exactly two starter species — Lactobacillus delbrueckii subsp. bulgaricus and Streptococcus thermophilus — at around 42 °C for a few hours. Both are fermented dairy, but they are different projects: yogurt is a controlled two-strain fermentation; kefir is a whole microbial community that ferments, reproduces, and carries on working after you buy it.

That community is the reason kefir tops the parent topic's ranking. Reviews of kefir microbiology consistently describe a crowd: lactic acid bacteria (Lactobacillus and Lactococcus species), acetic acid bacteria, and yeasts such as Kluyveromyces and Saccharomyces — 30 or more species in many analyses, with some studies reporting over 50 (Bourrie et al., Front Microbiol, 2016). More species is not automatically more health benefit — that is the honest caveat — but it does mean a more resilient fermentation, and a live bottle that keeps its microbiology all the way to your glass.

The Trial Data, Read Carefully

The flagship study is a randomized controlled trial of 22 adults with metabolic syndrome, published in Nutrients in 2019 (Bellikci-Koyu et al.). Participants drank 180 mL of plain kefir daily for 12 weeks; the control group drank the same volume of unfermented milk. The kefir group showed an increased relative abundance of Lactobacillus in their gut microbiota and reductions in several circulating inflammatory markers. Read carefully: 22 people, one study, modest effect sizes. It does not show kefir prevents or treats anything — it shows that drinking live kefir measurably moves gut biology in the direction you would hope for.

The second strand of trial evidence is older and, for daily life, arguably more useful. Hertzler and Clancy (J Am Diet Assoc, 2003) gave 15 adults with lactose maldigestion either kefir or milk and measured breath hydrogen — the standard marker of undigested lactose reaching the colon. Kefir cut that marker sharply and reduced flatulence and other symptoms relative to milk. Fermentation plus the live culture's own lactase activity does the work: kefir is not lactose-free, but it is dramatically easier to digest for many people who cannot handle milk.

What the file does not contain is a large kefir-versus-yogurt showdown. The ranking rests on these small trials plus the culture biology above, not on a head-to-head win. That is also the honest takeaway from the probiotics topic: strain identity and viability matter more than category names, and "kefir" on a label guarantees neither.

The practical upshot: in a live, plain version, kefir delivers a wider culture than yogurt per serving. In a pasteurized, sweetened version, it delivers neither. The difference between those two products matters more than the difference between kefir and yogurt.

The effects in the trials came from plain, unflavored kefir.
