Pregnancy roughly doubles circulating blood volume, raises cardiac output, and forces profound insulin resistance to feed the fetus. For most women the system absorbs the load without comment; when it does not, the complications are legible as a preview of vulnerabilities that aging will later reveal. Gestational Diabetes: The Metabolic Readout
Gestational diabetes marks a pancreas that ran out of headroom under pregnancy's insulin-resistant load. The most important downstream number is type 2 diabetes itself: meta-analyses find that women with gestational diabetes carry roughly a sevenfold higher risk of developing type 2 diabetes compared with women whose pregnancies were normoglycemic (Kramer et al., Diabetologia, 2019). Meta-analyses of cohort studies find women with PCOS carry approximately double the risk of coronary heart disease and stroke, independent of body weight (de Groot et al., Human Reproduction Update, 2011). What the Warning Buys You

It does not mean the later disease is inevitable; the same levers that move risk for everyone move it for these women, and several analyses suggest they may move it more. It does not require exotic intervention; it requires the ordinary stack applied early and consistently.
