Why Interactions Appear on No Label

Under DSHEA — the framework the label-is-not-proof page works through — a manufacturer never has to study how a product behaves alongside a prescription drug before selling it. Regulators learn about interactions from case reports, after-the-fact pharmacokinetic studies, and adverse-event filings that arrive late and undercount; a systematic review of seven top-selling herbs found 128 case reports or series alongside 80 clinical trials — with most interactions resting on case reports and plausible mechanism rather than controlled study (Izzo & Ernst, Drugs, 2009). The practical consequence: the pharmacy interaction screen sees only what you disclose — and many users disclose nothing, so the safest assumption about any medication list is that it is incomplete.

One distinction this topic repeats: verification is about the bottle, not about you. A USP or NSF seal means the contents match the label — it says nothing about whether the substance belongs in a body taking warfarin, or whether it helps anyone at all. Surgery: The Pre-Operative Conversation

Anesthesia is a drug interaction by design — dozens of agents tuned against your physiology in real time. A widely cited JAMA review of herbal medicines and perioperative care flagged direct effects on bleeding (garlic, ginkgo, ginseng), sedation depth (kava, valerian), heart rate and blood pressure (ephedra), and transplant-relevant immune activity (echinacea), alongside St John's wort's enzyme induction (Ang-Lee et al., JAMA, 2001). The exposure cuts both ways: some products are tapered rather than stopped outright, and that review's own table gives agent-specific withdrawal windows rather than one blanket rule. The windows quoted most often — most herbals held one to two weeks out — trace to anesthesia-society patient guidance and local pre-op protocols, not to a single binding national guideline, and practice differs between centres. When a Supplement Distorts the Lab Result

A subtler interaction happens before diagnosis: high-dose biotin — hair-skin-nail products can contain up to 650 times the recommended daily intake — interferes with immunoassays for troponin (the heart-attack marker) and thyroid hormones. The FDA has warned that falsely low troponin has contributed to at least one missed diagnosis and death (FDA safety communication, 2019). The fix is disclosure: tell the person drawing blood about high-dose biotin before the test, and expect guidance to hold it for a day or more. A thyroid panel run during high-dose biotin has steered people toward treating a thyroid problem they did not have.

Liver and Kidney Warning Signs

Supplement-associated liver injury is the harm category with the steepest consequences. In the US Drug-Induced Liver Injury Network's prospective cohort of 899 confirmed cases, herbal and dietary supplements accounted for 16% — 145 cases — and that share rose from about 7% to 17% across enrollment, though the authors caution that referral patterns may explain part of the climb (Chalasani et al., Hepatology, 2015). The timeline is what to internalize: injury usually announces itself within weeks to a few months of starting a product — the window people attribute to "adjusting to it" — and a minority appear only after a year or more, so duration alone proves nothing.

Two honesty notes. Attribution is genuinely hard: a new symptom that follows a new supplement is an association, not a diagnosis — but association is a good enough reason to stop the product while the workup happens. And most supplement adverse events on record are mild; the goal of this page is routing speed for the rare serious ones, not fear of the category.
