Resveratrol: The Bioavailability Problem

Resveratrol is the molecule that made red wine a health food in the public imagination — a grape-skin polyphenol that extended life in yeast and worms and seemed to explain the French paradox. Two decades of human trials later, the honest summary is shorter: the dose in wine is trivial, the dose in capsules mostly never reaches your bloodstream intact, and the largest pooled analysis of the blood-pressure trials — 17 of them, 681 people — still found no significant benefit overall.

Where the Story Came From

The resveratrol story begins with an observational puzzle, not a trial. French diets ran rich in saturated fat, yet French heart-disease rates ran lower than expected — a mismatch journalists named the French paradox, and a 1991 television segment pinned it on red wine. Resveratrol, a compound grape skins produce under stress, was in the bottle; laboratory studies showed it activating sirtuins and extending life in yeast, worms, and short-lived fish, and a supplement category was born. A widely publicized 2006 mouse study reported metabolic improvements in overweight mice, and sales went vertical.

Every step of that origin story deserves skepticism: the paradox is ecological, wine drinkers differ from non-drinkers in confounding ways, and the lifespan work was done in organisms a thousand times simpler than you. None of that disproves resveratrol; it means the marketing ran a decade ahead of the evidence — which is why resveratrol closes our audit of Longevity Supplement Candidates as its cautionary tale.

What the Human Trials Showed

Blood pressure was the most testable claim, so it collected the most data. The first reference point is a meta-analysis of six randomized controlled trials totaling 247 participants (Liu et al., Clinical Nutrition, 2014). Across all six trials, resveratrol produced no significant reduction in either systolic or diastolic blood pressure. When the authors isolated the trials using at least 150 mg per day, systolic pressure fell by an average of 11.9 mmHg (95% CI −21 to −2.8) — a number that would be remarkable if it rested on more than a few small, short trials. The authors' own conclusion was a request for larger and better-designed studies, which is researcher language for "we cannot tell you yet."

A larger pooled analysis partly answered that request. Seventeen randomized trials and 681 participants (Fogacci et al., Critical Reviews in Food Science and Nutrition, 2019) put the overall systolic effect at −2.5 mmHg, 95% CI −5.5 to 0.6 — not significant, but an interval wide enough to hide a small real benefit. Significant drops appeared only in the subgroups taking 300 mg per day or more and in people with diabetes.

Treat subgroup findings as hypotheses, not recommendations. Subgroups chosen after the fact are where false positives live, and even 681 participants is a small base to split. Blood pressure itself is a biomarker, not a measured health outcome. The Dose Arithmetic

Before the pharmacology, do the arithmetic. A glass of red wine delivers well under a milligram of resveratrol — measured wines have ranged from roughly 0.2 to 6 mg per litre, and even the richest put a 150 mL glass below 1 mg (Gu et al., 1999). The compound is a trace constituent of grape skin. The trials that produced even tentative signals used 150 mg to 500 mg per day: a gap of at least 150-fold between "the French paradox in a glass" and "the dose researchers study." Matching a trial day by drinking would take dozens of bottles — the marketing story as poisoning protocol. Whatever red wine does or does not do for health, it is not delivering pharmaceutical resveratrol.

The Bioavailability Wall

Suppose you skip the wine and take the capsule. The second problem is waiting in your intestine. The key pharmacology study (Walle et al., Drug Metabolism and Disposition, 2004) gave six volunteers a labeled 25 mg oral dose and tracked what survived. The answer: the compound is well absorbed — at least 70% of the dose crossed the gut wall — but rapidly and extensively metabolized, sulfated and glucuronidated by the gut and liver before it can circulate. Unchanged resveratrol in blood peaked below 5 ng/mL; parent compound and metabolites together peaked near 490 ng/mL. The molecule on the supplement label is not the molecule your tissues meet.

The safety study shows the wall stands even at high doses. Sergides and colleagues (Experimental and Therapeutic Medicine, 2015) gave 15 healthy volunteers a single 500 mg tablet and found it safe and well tolerated: free resveratrol peaked at 71 ng/mL against 4,084 ng/mL for its glucuronide. There is a genuine question underneath: some researchers argue the sulfate metabolites are not waste but a circulating reservoir the body can convert back. That debate is unresolved: the honest summary is not "activated longevity genes" but "the body sees mostly conjugates." One consequence is unambiguous: cell-culture studies bathing tissues in pure resveratrol test a situation human plasma never reproduces.

Side Effects and Watch-Items

⚠️ When this becomes clinician territory

If you take blood-pressure medication, a statin, an immunosuppressant, or any carefully titrated drug, adding hundreds of milligrams of a CYP3A4-inhibiting compound is a decision for the person who prescribed them — and so is stopping anything. Where That Leaves Resveratrol

It is worth being precise about what has and has not died. Resveratrol remains an interesting molecule: the metabolite question is open, and better formulations or longer trials could still change the picture. What died is the shortcut — that a trace wine constituent, or a capsule replicating it, is a validated longevity intervention. The industry sells the cell-culture story; the human dossier is null meta-analyses, a pharmacology that dismantles the molecule, and dose arithmetic that makes the wine framing absurd.
