TOFI: Thin Outside, Fat Inside
Some of the highest-risk people in metabolic medicine have a body mass index in the "normal" range. TOFI — thin outside, fat inside — names the pattern in which a slim frame hides a dangerous visceral and ectopic fat load. The body mass index sorts people by weight for height and nothing else. It cannot see where fat sits, how much muscle sits under it, or what the blood panel says — which is why two people at BMI 23 can have opposite metabolic profiles. The Concept's Lineage
The idea is older than the acronym. Where the Risk Concentrates
In imaging studies of normal-weight adults, the metabolic damage concentrates in the deep and ectopic depots rather than under the skin — the TOFI pattern in its original MRI formulation (Thomas et al., Obesity, 2012). The TOFI pattern is not distributed evenly across populations. In the M-CHAT imaging study, at matched body size South Asian and Chinese adults carried measurably more visceral fat than Europeans — the South Asian participants' visceral area was roughly 40% higher at the same waist circumference (Lear et al., Am J Clin Nutr, 2007).
