The Measurement Problem

The gut barrier is inside a living person, changing hour to hour, and it refuses to be photographed. Every method we have for assessing it is indirect — and the commercial tests are the most indirect of all. This page walks the actual toolkit: what each marker really measures, which ones earn their place, and what a "leaky gut" test result does and does not mean.

Why a Leak Is Hard to Measure

Start with the structural problem. The barrier you want to assess is a single cell layer wrapped around a 9-meter tube inside your abdomen. You cannot biopsy it routinely, you cannot watch it work in real time, and its behavior changes within hours of a meal, a workout, or a stressful morning. Every available test is therefore a proxy: you administer a probe and infer barrier behavior from what ends up where. Each proxy has its own blind spots, and the honest literature is explicit that no method has been accepted as a clinical diagnostic for "leaky gut" as a free-standing condition — the field's own reviews say this plainly (Camilleri, Gut, 2019). That single sentence explains most of the marketplace confusion: there is a real phenomenon and a real toolkit, but no approved test, no agreed threshold, and no reimbursement code. Into that gap, commercial testing has poured.

The Dual-Sugar Test: The Research Workhorse

The method with the deepest track record works by exploiting the size logic from the previous page. You drink a solution of two sugars that the body cannot metabolize. Mannitol is small and crosses the barrier through cells, so it reports how much surface area is working. Lactulose is large and can only slip between cells when junctions are loose, so it reports the leak specifically. Both end up in urine; the ratio of lactulose to mannitol corrects the leak signal for surface area, transit, and kidney clearance — which is why the ratio, not either sugar alone, is the metric. The approach goes back decades and remains the standard research measure of small- intestinal permeability. A 2021 validation from the Mayo Clinic established normal ranges in healthy adults using a standardized oral test, including a colonic component (Khoshbin et al., Gastroenterology), which moved the test from "research only" toward practical use. Honest limits remain: protocols vary between labs, a single test is still a snapshot, and the test is mostly used in clinical research and specialist practice — not in routine medicine, and not as a wellness screening tool.

The Zonulin Problem

Zonulin is the molecule every commercial panel centers on — and it is also the weakest link in the chain. The issue is not the biology (zonulin is real; see the definition page) but the assays. Commercial zonulin ELISA kits have been shown to measure different things from one another, and the proteins they detect do not always correspond to the zonulin characterized in the original research. Fasano himself — the discoverer — published a pointed commentary titled "Zonulin measurement conundrum: add confusion to confusion does not lead to clarity" (Gut, 2021), warning that values from commercial kits should be interpreted with extreme caution. Stool zonulin, sold in direct-to-consumer gut panels, is even less standardized than serum zonulin. The practical upshot: a borderline zonulin value on a commercial report is not evidence about your barrier. It is evidence that you bought a test.

🧪 How to read a commercial "gut health" panel

Most panels bundle several markers — zonulin, calprotectin, sometimes LPS antibodies — and print a verdict. A few rules keep you from being had: ask which study validated the panel's reference ranges (usually none); treat calprotectin seriously (it is a real inflammatory marker with clinical use in inflammatory bowel disease, and an elevated value belongs in a doctor's hands); and remember that no panel is accepted for diagnosing leaky gut, because the diagnosis does not exist in that form. If symptoms are driving the purchase, the higher-value test is the disease-specific one your clinician orders.
