Both Mediterranean arms showed roughly a 30 percent lower relative risk of major cardiovascular events over about five years — composite events, not blood pressure alone. Blood pressure outcomes were secondary, and the event reduction is the finding that has stood up.
The honest caveats: PREDIMED did not compare the Mediterranean pattern to DASH; it compared both to a low-fat control. Its participants were at high cardiovascular risk, not a general population. And relative-risk language overstates what an individual should expect — the absolute event rates were low, so the absolute benefit was smaller than the 30 percent framing implies.
