Thyroid & Weight: The Honest Math
"My thyroid made me gain forty pounds" is one of medicine's most durable folk beliefs, and it survives because half of it is true: thyroid hormone really does set the rate at which the body burns energy. But the measured effect is far smaller than the folklore — an untreated underactive gland typically adds five to ten pounds, much of it retained water rather than fat. Start with what is true: thyroid hormone is one of the body's main regulators of thermogenesis and a principal determinant of basal metabolic rate (Kim, Thyroid, 2008). What Hypothyroidism Actually Puts on the Scale
The textbook estimate for overt hypothyroidism is a gain of roughly five to ten pounds — real, noticeable, and much smaller than almost anyone expects. The composition is the surprising part: a large share is not fat. Heavier bodies appear to raise the setpoint slightly; association is not a one-way causal arrow.
The Other Direction: Hyperthyroidism Burns the House
An overactive gland produces the weight change people wish they had — and none of the value. In thyrotoxicosis the metabolic rate runs high, appetite surges to chase it, and when appetite loses the race the body catabolizes whatever is available: fat, but visibly muscle too — part of why overactive glands present with weakness alongside weight loss (De Leo, Lee & Braverman, Lancet, 2016). And everyone treated for hyperthyroidism should expect the scale to come back up: the rebound is the body recovering, not the treatment misfiring.

The Settling: Why Treatment Moves the Scale Once
Treatment changes weight exactly one time, then normal physiology resumes. For hypothyroidism started on levothyroxine, the water weight leaves over the first weeks to months — a few pounds — and after that the drug does nothing further for weight. For hyperthyroidism the settling is bigger and better documented. Birmingham's series followed 162 consecutive patients after treatment for a mean of two years: average gain 5.4 kg (about twelve pounds), more regain in patients who had lost the most weight before diagnosis, and an average 10.3 kg in the thirteen patients treated with thyroidectomy (Dale et al., Clinical Endocrinology, 2001). A meta-analysis of seventeen studies covering 3,164 thyroidectomy patients puts the average gain at 2.13 kg overall, rising to 5.19 kg when the operation was for hyperthyroidism (Huynh et al., JCEM, 2021).
Why the overshoot? During the overactive phase appetite runs hot to match a furnace; treatment douses the furnace quickly, appetite takes longer to reset, and some people sail past their pre-illness weight while it catches up. Knowing the number in advance is the intervention: the regain is expected and bounded. The settling period is roughly six months; weight still climbing hard after that deserves a review of both habits and labs.
Why Levothyroxine Is Not a Weight-Loss Drug
The corollary people want — more thyroid hormone, easier weight loss — fails in trials and fails dangerously. The cleanest test randomized 138 adults on stable levothyroxine to blinded dose changes targeting different TSH ranges for six months: no substantial differences in resting or total energy expenditure, body mass, or body composition across arms — while subjects confidently "preferred" the arm they believed was the higher dose (Samuels et al., JCEM, 2018). Pushing a dose for weight reasons is clinician territory, full stop.
The numbers behind that failure are worth knowing. In trial data, starting levothyroxine for overt hypothyroidism produces an average regain of roughly five kilograms — the water coming back, not new tissue — and meta-analytic estimates across treatment studies land near two kilograms. People hoping the prescription would also melt fat see none: at replacement doses, energy expenditure simply returns to normal, and appetite returns with it. Doses pushed above replacement add bone and cardiac risk for zero weight benefit — the reason every guideline statement on the topic is a warning. When Weight Change Warrants a Thyroid Check

⚠️ Weight change that is not a thyroid project
Rapid gain with swelling, breathlessness, or a slowed pulse; rapid loss with vomiting, fever, or confusion; or any unexplained shift beyond five percent of body weight in under six months — these are clinician territory before they are internet territory. And if you take thyroid medication, never adjust your own dose to manage weight: dosing belongs with the clinician who reads your labs.
Reading Your Own Scale Honestly
Two habits keep the math honest. First, weigh during the same thyroid state: comparing a pre-treatment weight to a post-treatment one mixes water shifts with fat change. Second, expect settling: treatment restores normal metabolism, not your twenties — appetite normalizes as the catabolic or retained-water state resolves, and a few pounds of drift in either direction is physiology, not failure. If weight keeps climbing on stable, well-managed replacement, the honest next step is a general metabolic workup — the thyroid is usually the wrong suspect by then.

- 📉 "Can hypothyroidism explain my 30-pound gain?" Almost never by itself. The honest ceiling for an underactive gland is around ten pounds, largely fluid; a larger gain needs the ordinary explanations.
