The standing desk arrived with big promises: weight loss, a posture fix, an escape from sitting itself. The measured reality is smaller and more useful. Standing burns roughly eight to ten more calories per hour than sitting, may take the edge off low-back discomfort, and changes little else that clearly matters. The Calorie Ledger: What Standing Actually Burns
Start with the number that sells the least and explains the most. Saeidifard and colleagues pooled 46 studies covering 1,184 participants and measured the sitting-versus-standing difference directly: about 0.15 kcal per minute, or roughly nine extra calories per hour (European Journal of Preventive Cardiology, 2018). Sitting at a desk runs in the neighborhood of 80 kcal per hour; standing moves the meter into the high 80s to low 90s. That is the entire metabolic difference. For contrast, walking slowly on a treadmill desk roughly doubles the sitting rate, and one structured training session burns more in forty minutes than a week of standing arithmetic adds. The pooled difference was slightly larger in randomized comparisons and in men — but no subgroup lands anywhere near a meaningful calorie burn.
The Marketing Math, Checked
The same meta-analysis supplies the marketing its favorite line: replace six hours of sitting with standing, and a 65 kg person expends about 54 extra kcal per day — which, compounded for a year with energy intake held perfectly constant, equals the energy in roughly 2.5 kg of body fat. The arithmetic is real; the world it assumes is not. Intake is never held constant — appetite nudges upward after standing work, and a single banana erases six hours of the standing bonus. Field trials of sit-stand desks report small changes in sitting time, not body weight. A standing desk is a weak weight-loss instrument, and any listing promising desk-driven weight loss is selling the arithmetic, not the evidence. Discomfort: The Benefit That Holds Up Best
The strongest honest case for a sit-stand desk is comfort, not calories. In a randomized trial, Husemann and colleagues (Human Factors, 2009) had 60 participants do data-entry work seated or alternating sitting and standing: the sit-stand group reported fewer musculoskeletal complaints, with a small, nonsignificant slowdown in data-entry speed while standing. A later meta-analysis of the low back (Agarwal et al., Ergonomics, 2018) pooled the trials and found a small standardized reduction in discomfort — on a 10-point scale, roughly 0.3 to 0.5 points. Two cautions: the studied populations were mostly pain-free office workers, and the outcomes were self-reported discomfort over weeks, not diagnosed conditions. Treadmill and Cycling Desks: Real Movement, Real Trade-Offs
The active desks are where the calorie story changes. A pooled analysis of laboratory studies (Oye-Somefun et al., BMC Public Health, 2021) found treadmill walking at desk speeds added about 105 kcal per hour over sitting. In real workplaces the gains shrink but survive: a randomized trial with overweight office workers (Schuna et al., 2014) found shared treadmill desks added about 1,622 steps per day and cut sedentary time by 3.6 minutes per working hour. Laboratory studies find discomfort climbing within one to two hours of uninterrupted standing, and people without access to a chair start inventing ways to sit. Watch-items belong beside every benefit: new ankle or leg swelling, heaviness by mid-afternoon, worsening visible veins, or numbness and tingling are signals to take to a clinician rather than to solve with furniture — and known venous insufficiency deserves that conversation before long standing blocks. The dose-response looks like a valley, not a hill: all-sitting and all-standing both score poorly, and the comfortable middle is alternation.
⚠️ When the desk is the wrong experiment
A standing desk is furniture, not medicine. New leg swelling or pain, skin color changes around the ankles, veins that bulge or ache, or persistent numbness warrant a clinician's assessment — those symptoms have causes a desk swap will not touch. Why does the framing matter?
